Format: 18 Hole Shotgun – carts required
Schedule: 8:00am Start – check-in 7:00am
Game: Two Best Net Balls
Tournament Limit: 72 Players
Sign-Up Deadline: Thursday, July 7, 2016
At time of sign up, make CASH payment for:

• Green Fee and/or Cart Fee to Pro Shop
• Members: $13.00 cart fee
• Non-Members: $45.00 (green fee/cart fee)
• Employees/Ambassadors: $5.00 cart fee

$25.00 Minimum donation (Cash or Check).
Make check payable to Hospice of Santa Cruz County.
This donation will be collected day of event.
Unable to play, and would like to make a donation??
The Pro Shop will be accepting donations up to July 11th.
